The specification is mostly about the slab and the floor finish. A poured slab needs a box set before the concrete; a raised floor or a wood-floor retrofit needs a different product entirely. Cover style — flip lid, screw plug, carpet flange — is chosen for the finish and for whether the floor has to stay flat when the box is closed. Fire rating and scrub-water rating both come up in commercial work.
